ATTACHMENT "A" <br />SCOPE OF WORK <br />The Contractor shall perform the following: <br />• Facilitate person -centered plans for adults receiving DDA services and working <br />with a Kittitas County subcontracted employment and day service provider. <br />• Plans shall encompass a person's whole life, with identified dreams, goals, what <br />works, what does not work, proper supports, skills, achievements, and a timeline <br />toward the plan. <br />• Document plan and provide copy to the individual, family, contracted employment <br />provider and to Kittitas County within two weeks of plan facilitation. <br />• Coordinate and schedule plan facilitation with county contracted providers. <br />Kittitas County to provide list of current providers, which are subject to change <br />throughout the year. <br />• Assist employment providers in identifying individuals who would most benefit <br />from person center plan facilitated by provider. <br />• Facilitate and document plans through the contracted year (Maximum of 9 plans <br />total in the contracted year). Scheduling can be flexible and coordinated with <br />providers as long as plans are completed within contract year. <br />Professional Services Agreement <br />Page 19 <br />
