ATTACHMENT "A" <br />SCOPE OF WORK <br />The KCHN shall perform the following -- <br />Training to increase job related skills and knowledge in the provision of services to <br />people with developmental disabilities: <br />• Review and familiarize with most recent DDA Caseload and Cost Report. <br />• Review and familiarize with educational handouts via DDA and Informing <br />Families <br />• Attend relevant community meetings such as Early Intervention Network, meet <br />with partner agencies, and attend relevant conferences. <br />Community Information and Education to inform and educate the public about <br />developmental disabilities and related services: <br />• Work with partner agencies to increase awareness of DDA and related services <br />that are available. <br />• Work directly with families who may qualify for DDA and related services and <br />provide technical assistance, as needed. <br />• Distribute information within Kittitas County via flyer, outreach materials, social <br />media, etcetera to provide understandable education to individuals and families <br />about services. <br />• Participate in relevant events that reach individuals and families who may qualify <br />for DDA and related services. <br />Professional Services Agreement <br />Page 19 <br />
