We track all outreach efforts <br />weekly to ensure anyone experiencing homelessness is identified and given continued <br />opportunities to receive assistance. We work closely with the City Homelessness Task <br />Force to come alongside individuals identified as homeless with high vulnerability and <br />currently unengaged needing services. Intakes include an internal assessment <br />designed for each demographic we serve to determine each client's level of <br />vulnerability and to develop a stability plan best suited to the needs of each client. <br />Literally homeless households receive priority over households that are imminently at <br />risk. The CE system, operated by HopeSource, has all households complete a standard <br />assessment survey. Tools and assessments are designed to be easily understood by <br />youth and to reflect the participants' strengths, goals, risks, and protective factors. <br />The Kittitas County Continuum of Care Coordinated Entry System for homeless <br />individuals has been created to better maximize community resources and quickly <br />re-house homeless individuals or prevent eviction. Using a web-based entry system <br />that can be accessed from any computer or phone, the system is positioned to deliver <br />information to the entire housing provider network immediately in one step, avoiding the <br />need to go "door to door" for assistance. Agencies then communicate among <br />themselves to determine the applicant's best choice and prioritize services.
