SECOND MODIFICATION OF THE EXISTING PROFESSIONAL SERVICES <br />AGREEMENT WITH ELLENSBURG FAMILY MEDICINE dba COMPASSDIRECT <br />HEALTHCARE FOR THE KITTITAS COUNTY JAIL <br />This Agreement, effective as of the date signed below, is made by and between Kittitas <br />County (hereinafter "the County") and Ellensburg Family Medicine dba CompassDirect <br />Healthcare ("Contractor"). The County and Contractor agree as follows; <br />That the existing Agreement, previously entered between the parties on December 7, <br />2021 and modified June 312022, shall be additionally modified as allowed by section <br />12 (twelve) of that agreement. The purpose of this modification is to expand services to <br />include weekend coverage at the jail and increase the total amount paid to the <br />Contractor. The total amount payable to the Contractor will increase from $236,000 <br />annually, as reflected in the June 31,2022 modification, to no more than $310,000 - <br />reflecting a cost increase of up to $75,000. This increase will be funded by existing <br />grant awards and Its continuation contingent upon those awards being renewed. <br />Scope of work: <br />1. Assume responsibility of Suboxone med pass for the jail, and weekend <br />coverage of 1st med pass. <br />2. Triage any acute needs while on site providing aforementioned weekend <br />services. <br />All terms, conditions, attachments, documents, and exhibits, including the term of the <br />agreement as described in the original Agreement are incorporated herein by this <br />reference as fully as If set forth herein. <br />
