WA : I W Ill 9WAA <br />SCOPE OF WORK <br />In addition to providing all material and labor, the Contractor shall perform the following: <br />Contract with Kittitas Valley Healthcare Hospital to provide continuation of two Crisis Case <br />Managers to assist with behavioral health patients in the emergency department for a total <br />of 70 hours a week of coverage. <br />Submit a performance report to the 1 /loth of 1 % Board Clerk: <br />marie.marchand co.kittitas.wa.us on or before December 10, 2023. The report <br />shall include, at a minimum: <br />i. A narrative of project activities during the reporting period. <br />ii. A narrative of project successes during the reporting period. <br />iii. A narrative of project challenges during the reporting period. <br />iv. Number of people the project reached during the reporting period. <br />Attend meetings with Kittitas County Public Health Department staff, as needed, <br />throughout the project duration. <br />Professional Services Agreement <br />Page 13 <br />
