DocuSign Envelope lD: BF50DFC0-2E68-460F-9CDB-06D04707CD3D <br />Statement of Work. <br />This contract was won competitively, and contract incorporates by reference the Statement of Work WSCJTC <br />published in the Request for Proposal, which the Contractor's proposal specifically agreed to perform. <br />Contract reporting and lnvoice dates: <br />1. Due by March 15,2023. Outcome Report for the months of November 2022, <br />December 2022, January 2023, and February 2023. A report with updates <br />towards outcomes and data. A19 lnvoice and back up documentation needs to <br />accompany this report. <br />2. Due by July 10,2023. outcome report for months March, April, May, and <br />June 2023. A19 lnvoice and back up documentation needs to accompany this <br />report. ln addition, a final report summary that covers the length of the <br />contract and how the services met or did not meet your needs is required. <br />Outcome Expectations: <br />1. Trainings provided and usefulness of those training (survey results). <br />2. Document sheriff retention and reasons for leaving <br />3. Peer Support statistics <br />r Times they were contacted <br />o Number of times by each employee. Unplanned days of leave <br />o Number of injuries <br />o Complaints of attitude/unprofessionalism.r Suicide attempts decrease/increase <br />Exclusive Agreement. This contract, with its attachments and documents incorporated by reference, contains all of <br />the terms and conditions the parties agreed to. No other contract terms or conditions shall be deemed to exist or bind <br />the parties. The parties signing above confirm they have read and understand this entire Contract and have the <br />authority to enter into this Contract. WSCJTC and the Contractor may amend the contract by mutual written <br />agreement. <br />Fees and limitations. At any time, WSCJTC may direct the Contractor to suspend work on the contract, pay the <br />Contractor for hours expended before the suspension, and no further payments are due until WSCJTC directs the <br />work to resume. lf expenses are allowed, they are charged against the Contract Maximum Amount above. Travel <br />expenses, if authorized, shall never include Contractor's regular commute from home to the work site. <br />Payment. WSCJTC shall pay the Contractor for performance of the Statement of Work, in response to invoices <br />specifying hours worked or work completed but shall not pay in advance. Payments are made by Electronic Funds <br />Transfer using the bank routing information the Contractor provides. <br />lndustrial lnsurance Coverage. WSCJTC will report the Contractor to the Department of Labor and lndustries (L&l) <br />as a "non-employee covered worker' and will pay L&l insurance premiums. Any injuries the Contractor suffers in the <br />course of performing this contrac{ are covered by L&|. The Contractor and his/her physician should claim accordingly. <br />lf this contract authorizes subcontracting, the Contractor provides L&l coverage for any subcontract workers; WSCJTC <br />and the State assume no liability for them.
