<br />All recipients, suhecipients, successors, transferees, and assignees mLet acknowledge and agree to comdy wilh apgicable
<br />provisions gwerrfng DHS accessto records, ac@unts, documents, information, facilitiss, and slaf,
<br />l. Recipients musl cooperate with any DHs compliance reviews or comgnnce rrwestigatrors condrrcted by DHs.
<br />ll. Reciptents must give DH$ access to examire and copy records, accounts, ard other docurnents and sources of infornration
<br />related to the federal firnrrcial assbbnoe award and permit acoess to faoilities or personnel,
<br />lll. Rectpients must submit ttrnely, complete, and acourate reports to the appropriate DHS ofiicials and mainbin appropriate
<br />backup documentalion to supporl the repoils.
<br />lV Recipients must comply with all ottEr speclal reporting, data collection, ard evatualion requiremenb, as p{escribed by law,
<br />or delailed in program guidance.

<br />After the initlal submission, recipients are required to complete the tool orre every two (2) years if they have an active
<br />award, not everytirp an award is made. Recipients shoub subrnit the completed tool, rncludirg supporting nnte1als, to
<br />GivilRightsEvaluation@hq.dhs.gov. This tool olarifies the civil rights oH[atbrn ard related reporting reqrirennnts conlained
