Time and Mater'2I Work Order 7 7 <br />y,, <br />Scope of Work <br />Time and Material for TKE and third party vendor to drill and remove jack and casing, then install the following: <br />1 - 24" diameter steel casing using a Rail Mounted limited access indoor drill with various tools and casing in on <br />mobilization with 2 man crew on daily rate. <br />Daily Rate is $3,550.00 (8 -hr. straight time day - portal to portal) <br />One time fee - DBM $7,500.00 roundtrip mobilization of tools, equipment and one time jackshaft casing fabrication <br />fee. <br />One time cost - jack casing 24" diameter casting @ $89.00/If. <br />Associated materials and misc. expendables will be added at invoice cost + 20% overhead + 10% profit. <br />Please Note the following: <br />1. The above rates are T&M quotation and the anticipated duration is based on unosbrtucted drilling conditions, and is <br />solely an estimate. Estimated durations are NOT a guarantee. <br />2. Daily rate starts when DBM leaves their Federal Way yard and includes set up and tear down until we return to our <br />yard. <br />3. DBM fees do not included the cost of substance abuse testing or L£tl filing fees. <br />4. We will need to utilize some of the parking area for staging of misc. tooling, gang boxes, small fork lift, ecopans, etc. <br />We can coordinate this with a pre con meeting with the faccility manager to see where would be the best place before <br />we move in to minimize the impact to their operations. <br />5. TKE will get the elevator car to the top of the hoist way giving the highest pick point as possible - which is critical to <br />the speed of the drilling process. <br />6. After DBM speaking with TKE about the jack, - it sounds like the jack has been backfilled with concrete inside the <br />existing casing. DBM concerns ae that when they start pulling on the jack that the existing casing is goin gto come out <br />with it also. Once DBM removes the jack and the casing does come out, DBM will have to drill and set a new 24" <br />casing. There is a chance this will not happen but need to be prepaired if it does. If it does happen the additional cost <br />would be: see above - One time Jack shaft casing fee. <br />7. All oil within the jack will be removed by TKE <br />8. DBM will drill an ovesized open hole to the depth as needed, then set the new casing in sections welding them <br />together. If the soils will not stand open or there is too much water DBM may have to put a water/polymer slurry head <br />in the hole to stabilize the soils or possibly drive casing. Once the casing is set on location, DBM will back fill with the <br />drill spoils if suitable or pea gravel. <br />9. The best way to deal with the spoils removal if drilling is needed would be to us ECO Pans, they will deliver pans as <br />needed and take away spoils for disposal. If the soils are wet and a water/polymer slurry is used, we may need the <br />services of a vactor truck for the slurry removal as well. <br />10. TKE will bill elevator mechanic labor separately: Labor rate $409.50 hr per mechanic (Standard labor rate $585 <br />less 30%= $409.50) <br />DBM approximation is: 20 to 25 days +/- on site if a new hole is drilled. If DBM is able to remove the jack with out <br />removing the existing casing and existing casing is suitable to use, the cleaning out of the casing and jack removal <br />should be considerbly less. This price does not include the actual cost of rentals, spoils removal, contaminated <br />materials, backfill, water containment or disposal.
