(2) Clients in an employment program will be supported to work towards a living <br />wage. The cMls county <br />service Authorization and updated Planned Rates information will not <br />exceed the Client's DDA PSCP' <br />(b) The amount of service the Client receives should match with the CMIS <br />county service Authorization and updated Planned Rates information' <br />(4) All clients will have an individu alized plan to identify client's prefere.nces' <br />Minimum plan elements are outlined in the reference document "Criteria for <br />an Evaluation." A copy of the client's individualized plan will be provided to <br />their CRM, guardian and others as appropriate' <br />(5) Semi-annual progress reports that describe the outcomes of activities will be <br />provided by the p"rovider io the county and the cRM, guardian and others <br />as approprjate. ihe report will summarize the progress made towards the <br />Client's individualized goals. <br />(6) All clients will be contacted by their service provider according to client <br />need and at least once Per month. <br />(7) lf clients in lndividual Employment, Group Supported Employment, or <br />Prevocational services have not obtained paid employment at minimum <br />wage or better within six (6) months, the County will assure the following <br />steps are taken: <br />(a) Review the progress toward employment goals; <br />(b) Provide evidence of consultation with the family/client; and <br />(c) Develop additional strategies with the family/client, county staff, <br />employment support stafiand the case manager.
