EXHIBIT "A" <br />SCOPE OF WORK <br />The KCHN shall perform the following: <br />Utilize Community Health Workers to increase public awareness and knowledge <br />about Developmental Disabilities Administration (DDA) services specific to the 0 to <br />17 aged population and the benefits to early enrollment through in-person outreach, <br />outreach events, social media, paid advertising. <br />Utilize Community Health Workers to refer 0 to 17 aged children to DDA services by <br />assisting parents and guardians with the enrollment application process and provide <br />information about DDA eligibility resources. <br />KCHN personnel will coordinate with early intervention agencies, schools, childcares, <br />and other community agencies who serve children to train and increase capacity for <br />early intervention and identification of children who would benefit from DDA services <br />through systematic screening of children and referrals to DDA. <br />Professional Services Agreement <br />Page 13 <br />
