ATTACHMENT"A" <br />SCOPE OF WORK <br />In addition to providing all material and labor,the Contractor shall perform the following: <br />1.Client Eligibility:Client eligibility and service referral are the responsibility of the <br />DDA pursuant to Chapter 388-823 WAC (Eligibility)and chapter 388-825 WAC <br />(Service Rules).Only persons referred by DDA shall be eligible for direct Client <br />services under this Program Agreement.It is DDA's responsibility to determine and <br />authorize the appropriate direct service(s)type.Direct Client services provided <br />without authorization are not reimbursable under this Program Agreement. <br />2.Client Rights.The Contractor shall: <br />a.Ensure that a statementof Client Rights are posted in compliance with Section <br />388-850-050 WAC;and <br />b.Ensure that Client rights and client health and safety are protected in <br />compliance with Section 388-850-040 WAC and DDA Policy 5.06:Client <br />Rights. <br />c.Ensure protection from abuse:mandatory reporting per DDA Policy 5.13. <br />3.Credentials and Minimum Requirements. <br />Contractor shall ensure that Contractor meets the following requirements: <br />a.Fiscally Responsible:The Contractor must demonstratethe ability to safeguard <br />public funds including maintaining books,records,documents and other <br />materials relevantto the provision of goods and services. <br />b.Contractor shall provide a report and/or presentation of millage funds spent to the <br />County within 30 days of the end of the contract year. <br />c.Sufficient Policies and Procedures for Establishmentand Maintenance of <br />adequateinternal control systems:The County will maintain written policy <br />procedural manuals for information systems,personnel,and accounting/finance <br />in sufficient detail such that operations can continue should staffing change or <br />absences occur. <br />d.Background/Criminal History Check:A background/criminal history clearance is <br />required every three years for all employees(including DD program County <br />Professional Services Agreement <br />Page 19
