EXHIBIT "A" SCOPE OF WORK The Contractor shall employ one or more Community Health Workers /Promotoras de Salud,who will work to decrease barriers to accessing COVID-19 vaccination for Kittitas County's Latinx and Spanish-speaking households.The position requires courteous and cooperative interaction with all department staff,County,State,and Federal agencies, and the general public,and will be expected to provide assistance in other areas of Public Health as needed.All employees work under the guidance of locallstatelfederal regulations. Essential Functions: •Improve COVID-19 testing and vaccination access and uptake in local Latinx and Spanish-speaking communitiesby: •Building relationshipswith community leaders and organizations •Identifying and implementing outreach strategies to reduce barriers to testing and vaccination •Increasing communityCOVID-19 knowledge •Developing effective relationships with community members and community partners •Assisting with care coordination services related to vaccine access and barrier reduction •Working in partnership with KCPHD staff •Including information on other recommended vaccinations,including influenza and childhood immunization series. OTHER FUNCTIONS: ·Respond to Public Health related inquiries from the public as needed. •Maintain confidentiality as described in agency,federal,and state required policies. This job description is a summary of the primaty functions and expectationsfor this position.Additionalduties may be assigned as required. MINIMUM QUALIFICATIONS: •Proficiency in verbal and written Spanish. •Insight and knowledgeof cultural norms in local Latinx communities. •At least one (1)year of professional or volunteerhealth,social service,or related experience. •Valid Washington State driver's license.
