DocuSignEnvelopeID:FF14B287-E90C-42B9-8A0F-A14FA00E82AFvi.Provideorarrangefor,timelyAppleHealth/MedicaidbillingtrainingassistancetoABCDofficestaffandproviders,asneeded;vii.Atminimumannuallyupdatedentalsociety(orlocaldentists/studyclubsincountieswithoutasociety)ontheABCDProgramandsupporttheircontinuedparticipationintheprogramandencouragerecruitmentofnewAppleHealth/Medicaidproviders;andviii.SupporttheDentalChampion(s)participationinmeetingsandactivitiesnecessarytoeffectivelyconductAppleHealth/Medicaidprovideroutreach,recruitmentandtraining,includingfinancialsupportofattendance(travel,lodging,etc.)inDevelopmentDay.ix.ParticipateintheannualstatewideABCDChampionDevelopmentDaymeetingtoremaincurrentwithnewclinicalpracticesandopportunities.Meet,network,andshareknowledgewithotherchampionsregardingprogramroadblocksandsuccesses.1.ExtendinvitationtoABCDDentalChampiontoattendtheannualABCDChampionDevelopmentDaymeeting.2.ABCDChampiontravelreimbursement,ifany,toattendDevelopmentDay,isincludedinthetotalcompensation.f)IdentifyandrecruitprimarycaremedicalproviderstoparticipateinAppleHealth/MedicaidasABCDcertifiedproviders,securetheirtrainingthroughArcoraFoundationinpreventiveoralhealthcaretechniques(FamilyOralHealthEducation,fluoridevarnish,etc.)andbuildtheirroleinreferringAppleHealth/Medicaid-eligiblechildrentotheABCDProgram.g)Participateinallthree(3)statewideABCDCoordinatorsgroupmeetingstoremaincurrentwithABCDpolicies,practicesandopportunities.ProgramswhichdonotmeetthisannualcontractrequirementwillbesubjecttocontractreviewbyHCAandpotentially,tolossofthiscontract.h)IdentifyanABCDCoordinatorwithinthecontractingorganizationwhowilldevelopandmaintainadeskmanualthatoutlinestheexpectedABCDcontractualdeliverablesandhowthecontractormeetseachdeliverable.TheCoordinatorwillutilizethismanualtofulfillthecontractualrequirementsandtoorientnewleadstaffwithintheorganizationtotheABCDprogram.i)IftheContractor'sCoordinatorvacatestheposition,theContractormustnotifytheHealthCareAuthoritywithintwoweeksofthecoordinator'sdeparture,and;i.SharetheContractor'sdevelopedworkplanthatoutlineshowtheexpectedcontractdeliverableswillbemet;ii.IncludetheHCAandtheStateManagingPartner,ArocraFoundation,inthehiringprocesstofindanewcoordinator;iii.SharewithHCAthecontactinformationofthenewlyhiredorappointedCoordinator;iv.CoordinatewithHCAtoassureasmoothtransitionoftheexpectedcontractedworkdeliverables,includingparticipationinprogramorientationwithHCAandotherstatepartnersand;v.ABCDCoordinatornewhiremustreasonablymeettheexpectationsasidentifiedinExhibitDABCDCoordinatorPerformanceExpectationsandAbilities.Page6of20HCAContractNo.:K2747-04
