ThisProgramAgreementbetweentheCountyandtheStateofWashingtonDepartmentofSocialandHealthServices(DSHS)isherebyamendedasfollows:1.TheTotalMaximumContractAmountisherebyincreasedintheamountof$630,642foranewContractAmountof$1,261,2842.Section1.DefinitionsSpecifictoProgramAgreement,itemso,r,anduareherebyreplacedwiththefollowinglanguage:o."EmploymentOutcomePayment"meansapaymenttoprovidersfortransitionstudentsbornbetweenspecificdates;havehighacuity;areauthorizedforIndividualEmployment,andstudentobtainsacompetitiveintegratedjobwithinspecifictimeframes.Ifthejobisaminimumoftenhoursofworkperweekanadditionalamountwillbeincludedinthepayment.r."JobFoundationReport"meansadocumentderivedfromemploymentreadinessactivitiesperformedbystudentswhoarebetweenages19through20thatidentifiesactionablenextstepsforemployment.TheemploymentserviceprovidersdevelopingtheJobFoundationreportwillbesupportingstudentswithemploymentactivitiesonaverage35hours.u."QualityAssurance"meansanadherencetoallProgramAgreementrequirements,includingDDAPolicy6.13,ProviderQualificationsforEmploymentandDayProgramServices,CountyGuidelines,andtheCriteriaforEvaluation,aswellasafocusonreasonablyexpectedlevelsofperformance,qualityandpractice.3.Section4.CredentialsandMinimumRequirements,itemh.isherebyreplacedwiththefollowinglanguage:h.QualifiedServiceProviders:TheCountyassuresthatallserviceprovidersmeetqualificationsasoutlinedintheDDAPolicy6.13,ProviderQualificationsforEmploymentandDayProgramServices.4.Section6.StatementofWork,itemsk.(4),(5)and(11),t.andu.areherebyreplacedwiththefollowinglanguage:(4)AllClientswillhaveanindividualizedemploymentorCommunityinclusionplantoidentifyClient'spreferences.Minimumplanelementsareoutlinedinthereferencedocument"CriteriaforEvaluation".AcopyoftheClient'sindividualizedplanwillbeprovidedtotheClient,theirCRM,guardian,andothersasappropriate.(5)Six-monthprogressreportsdescribingtheprogressmadetowardsachievingClient'sgoalwillbeprovidedbytheserviceprovidertotheCRM,participant,and/orguardian,ifany,within30daysfollowingthesix-monthperiod.ThereportwilsummarizeactivitiesandoutcomesmadetowardsClient'sindividualizedgoal(s).(11)ForGroupSupportedEmployment(GSE),Clientsmusthavepaidwork.Thetotalnumberofdirectservicestaffhoursprovidedtothegroupshouldbeequaltoorgreaterthanthegroup'scollectiveamountofauthorizedservicehours,ifthedirectservicestaffhoursarelessthanthecollectiveamount,thentheproviderwillbereimbursedonlyforthenumberofhoursstaffactuallyprovide.t.PartnershipProject.(1)AJobFoundationdocumentwillbecompletedperguidelinesforeligiblestudents.EligiblestudentsareDDAClientswhowerebornbetween:DSHSCentralContractServices1769CPContractAmendment(12-10-21)Page2
