COUNTY JAIL <br />This Agreement, effective as of the date signed below, is made by and between Kittitascgunty (hereinafter'the County") and Ellerisuurg ramity nlleoicine DBA comfasroirect("Contractor"). The County and Contractor agree as follows: <br />Il'llhq existing Ag.reement,.previously entered between the parties on December 7rh,2021, shall be modified as allowed by section 12 (twelve) oithat agreement. Theptlrpose of this Modification is to allow for a nurse to be hired to he'ip witfr coveiage atthe jail due to the inorease of services and increase tne totaiamount paid to theConhactor'. Hiring a nurse to help with coverage at the jail will allow the Contractor toprovide additional services for the jail (see belJw for additional services tnat wouiO oeadded). The total cosl to the Contiactor will increase from no greater than 9100,000annually to no greater than $235,000 annually, reflectjng an esitimated cost of theadditional personnel and services of 9135,00b, fnis wiiiUe paid from the same'funO.Scope of work: <br />1. Distribute medicine to the inmates at their designated times2. Chart review and audit <br />3. Triage of any acute needs4. Records requests and reviews5' Assistance-with implementation of the elechonio medical records system6. Provide in-facility lab draws (also done by other staff from their offtieti.
