Should an individual rnake a request to Covered Entity For an accountittg <br />of disclosures of his or her PHI pursuant Lo 45 CFR $ 164.528, Business Associate agrees <br />to prornptly provide an accountirrg, as specihed under 42 U.S.C, $ 17935(c) (l) and 45 <br />CfR $ 164.528. of disclosures of PHI that have been made by the Business Associate <br />acting on behalf of the Covered Entit_v. The accounting shall be provided by the Busincss <br />Associate to the Covered Entity or to lhe individual, as directed by the Covered Entity. <br />I. Amendments: Nlake any amendments to PHI that the Covered Entity directs or agrees to <br />pursuant to 45 CFR g 164.5?fr at the request of Covered Entity, within five (5) business <br />days of written rcquest by Covered Entity. <br />J. Qther: To the extent the Business Associate is to cany out one or mole ol the cuvered <br />entities obligations under Subpart E of 45 CFR 164, contply lvith the requitements of <br />Subpart E that apply to the covered entity in the pertbnnance of such obligations. <br />tu.PERVIITTED US AND DISCLOSURES BY BUSI ss AssoclATE <br />A.
