<br />KCHD ORDINANCE 1999- 24 SOLID WASTE REGULATIONS <br />(10) Every permit application received by the Health Officer shall be on a format prescribed by the <br />Health Officer and shall contain specific requirements necessary for the proper operation and <br />development of the permitted site or facility, including the requirement that final engineering <br />plans, specifications and other reports be submitted for approval to the Health Officer. <br />(11) The Health Department must file all issued permits with Ecology no more than seven (7) days <br />after the date of issuance. <br />(12) The owner or operator of a facility shall apply for renewal of the facility's permit annually. <br />The Health Officer shall annually: <br />(A) Review the original application for compliance with these regulations and such additional <br />information as required in Section VI.A.2.c.; <br />(B) Review information collected from inspections, complaints or known changes in the <br />operations; <br />(C) Collect the renewal fee; <br />(D) Renew the permit; and <br />(E) File the renewed permit with Ecology no more than seven (7) days after the date of <br />issuance. Ecology shall review and may appeal the renewal as set forth in RCW 70.95.185 <br />and RCW 70.95.190. <br />b. General Application Contents for Permits for New or Expanded Facilities. Except as provided for <br />in Section VI.C., all permit applications for facilities subject to these regulations, shall include, but <br />may not be limited to, the following: <br />(1) A general description of the facility; <br />(2) The types of waste to be handled at the facility; <br />(3) The plan of operation required by WAC 173-304-405 (2); <br />(4) The form used to record weights and volumes of each type of waste stream accepted as <br />required by WAC 173-304-405 (3) and stipulated by the approved Kittitas County <br />Comprehensive Solid Waste Management Plan; <br />(5) An inspection schedule and inspection log required by WAC 173-304-405 (5); and <br />(6) Documentation to show that any domestic or industrial wastewater treatment facility, such as a <br />leachate collection and treatment system, is being reviewed by Ecology under Chapter 173- <br />240 WAC (Submission of Plans and Reports for Construction of Wastewater Facilities). <br />c. Application Contents for Existing Facilities Renewing Permits. <br />(1) All owners or operators of existing facilities shall renew permits on application forms specified <br />by the Health Officer. Previous information submitted to the Health Officer may be referred to <br />on the application forms. Changes in operating methods or other changes must be noted on the <br />application in order to be authorized by permit.
