PROFESSIONAL SERVICES AG REEMENT <br />This Agreement is made by and between Kittitas County (hereinafter'the <br />County") and Community Health of Central Washington (hereinafter ;Contractor"). The <br />County and Contractor agree as follows: <br />General Conditions; Exhibit A (Scope of Work); Exhibit B (Compensation); <br />Exhibit C (Proof of lnsurance); Exhibit D (Consolidated Contract <br />CH18249between Kittitas County and the Washington State Department of <br />Health); Exhibit E (CYSHCN Quarterly MCHBG Reporting Form); Exhibit F <br />(CYSHCN Annual Reporting Form) <br />copies of which are attached hereto and incorporated herein by this reference as fully as if <br />set forth herein. <br />The following documents and/or laws are incorporated by reference as fully as if set forth <br />herein: <br />o HRSA - Title V Maternal and Child Health Services Block Grant Program <br />http://mchb. hrsa.qov/proqrams/tiflevg rants/index. htmlo Social SecurityAct Title V Laws: <br />http://www.ssa.qov/OP Home/ssacUtifl e05/0b00. htm <br />The term of this Agreement shall commence on the October 1st,2021, and continue <br />until December 31,.2021.
