StateofWashingtonDepartmentofEcologyPage11of22AgreementNo:SWMCLCP-2123-KiCoSW-00045ProjectTitle:KiCoSW-00045LitterandIllegalDumpCleanupandPreventionProjectRecipientName:KITTITASCOUNTYSOLIDWASTEFundingDistributionSummaryRecipient/EcologyShareFundingDistributionNameRecipientMatch%RecipientShareEcologyShareTotalKiCoSWLitterandIllegalDump0.00%$0.00$70,404.00$70,404.00CleanupProjectTotal$0.00$70,404.00$70,404.00AGREEMENTSPECIFICTERMSANDCONDITIONSN/ASPECIALTERMSANDCONDITIONSGrantadministrativecostsarelimitedto10percentofgrantapprovedexpenditures,excludingToolsandTrucks.AdministrativecostsforToolsandTrucksarenotallowedunderthisgrantprogram.RECIPIENTmustsubmitwithinthirty(30)daysaftertheexpirationdateofthisAgreement,allfinancial(includingpaymentrequests),performance,andotherreportsrequiredbythisAgreement.ECOLOGYshallhavetherighttodenyreimbursementofpaymentrequestsreceivedafterthisdate.EcologywillconductariskassessmentofallCommunityLitterCleanupProgramrecipients.ThelevelofriskdeterminesthelevelofoversightrequiredbyEcologythroughoutthebiennium.Iftherecipient'sperformanceorprojectcircumstanceschange,Ecologymayreassessriskandnotifytherecipientofanychangestoadministrativerequirements.GENERALFEDERALCONDITIONSIfaportionorallofthefundsforthisagreementareprovidedthroughfederalfundingsourcesorthisagreementisusedtomatchafederalgrantaward,thefollowingtermsandconditionsapplytoyou.A.CERTIFICATIONREGARDINGSUSPENSION,DEBARMENT,INELIGIBILITYORVOLUNTARYEXCLUSION:1.TheRECIPIENT/CONTRACTOR,bysigningthisagreement,certifiesthatitisnotsuspended,debarred,proposedfordebarment,declaredineligibleorotherwiseexcludedfromcontractingwiththefederalgovernment,orfromreceivingcontractspaidforwithfederalfunds.IftheRECIPIENT/CONTRACTORisunabletocertifytothestatementscontainedinthecertification,theymustprovideanexplanationastowhytheycannot.2.TheRECIPIENT/CONTRACTORshallprovideimmediatewrittennoticetoECOLOGYifatanytimetheRECIPIENT/CONTRACTORlearnsthatitscertificationwaserroneouswhensubmittedorhadbecomeerroneousbyreasonofchangedcircumstances.3.Thetermscoveredtransaction,debarred,suspended,ineligible,lowertiercoveredtransaction,participant,person,primarycoveredtransaction,principal,proposal,andvoluntarilyexcluded,asusedinthisclause,havethemeaningsetoutintheDefinitionsandCoveragesectionsofrulesimplementingExecutiveOrder12549.YoumaycontactECOLOGYforassistanceinobtainingacopyofthoseregulations.TemplateVersion12/10/2020
