5. Purpose of Use: Classroom for KCPHD/UW research project. <br />6. Rent: $300/month <br />7. Special conditions: KCPHD will be responsible for cleaning the classroom. Empty <br />trash in large can provided outside. KCPHD program manager will have access to <br />Classroom 2 and restrooms. <br />8. Termination: <br />This agreement shall continue for the agreed term, or until either party terminates this <br />agreement by giving at least thirty (30) days written notice. User shall vacate the <br />Premises immediately upon termination of the agreement. <br />9. Notices and Contact Information: <br />All notices pertaining to this agreement shall be made in writing to the parties <br />at the following addresses and phone numbers: Church office (EUMC, 210 N. Ruby <br />Street, Ellensburg, WA 98926, 509-962-9257). Building Use Managers: <br />(509-899-5028, Nancy Goodloe) or (509-607-1342, Karen Lasik). <br />10. Conditions of Use: <br />a. Rental Fees shall be submitted by the 1 st week of every month or the comple- <br />tion of the contractual terms above. Please remit to payments to EUMC in care of <br />Bill Beals, Treasurer. <br />b. Church policy requires no smoking or firearms on the premises. <br />c. Alcoholic beverages shall not be brought onto the premises unless the user <br />group has been given permission well in advance of the event from the Building <br />Use Managers. <br />d. The premises shall be a violence -free zone. <br />e. User shall not discriminate on the basis of gender, race, age, sexual orientation, <br />national origin, mental or physical disabilities, marital status or economic condition. <br />f. All children within the church building or grounds MUST be supervised by the <br />User group and not allowed to be in any areas of the building or its premises with- <br />out an adult supervisor. This practice is consistent with the "Safe Sanctuaries" for <br />children and youth policy adopted by this congregation. <br />The following guidelines of the Safe Sanctuary policy are particularly important: <br />L The Safe Sanctuaries policy typically requires no fewer than two adults to be <br />present at all times during a program or event involving children. The Kittitas <br />County Public Health Department is authorized to have a single staff person with <br />all required state background checks overseeing the children of clients without a <br />second adult present. <br />ii. At least one of the adults present should be competent/currently certified in <br />First Aid and CPR. <br />iii. The program staff must read the—Safe Sanctuary policy and agree to its terms <br />as mandated reporters of child abuse. If a case of child abuse is observed by <br />KCPHD Staff within their group, or by other groups on the church premises, while <br />
