PROFESSIONAL SE ES AGREEMENT <br />This Agreement is made by and between Kittitas County (hereinafter "the County") <br />and Yakima Valley Memorial HospitalAssociation, a Washington nonprofit corporation <br />located at 2811 Tieton Drive, Yakima, WA 98902 (hereinafter "Contractor"). The County <br />and Contractor agree as follows: <br />General Conditions; Attachment A (Scope of Work); Attachment B <br />(Compensation); Attachment C (Proof of lnsurance); Attachment D (Progr:am <br />Agreement between County and DSHS for Developmental Disabilities County <br />Services). <br />copies of which are attached hereto and incorporated herein by this reference as fully as if <br />set forth herein. <br />The term of this Agreement shall commence on the July 1 ,2021 and continue until <br />June 30, 2022.
