The report will summarize the progress made towards theClient's individualized goals. <br />(6) All Clients will be contacted by their service provider according to Client <br />need and at least once per month. <br />(7) lf clients in lndividual Employment, Group supported Employment, orPrevocational services have not obtained paid employment it rinirut <br />wage or better within six (6) months, the County will assure the following <br />steps are taken: <br />(a) Review the progress toward employment goals; <br />(b) Provide evidence of consultation with the family/client; and <br />(c) Develop additional strategies with the family/Client, county staff, <br />employment support staff and the case manager. Strategies may includeproviding technical assistance, changing to a new provid'-er, and/orproviding additional resources as needed to support the individual,spursuit of employment. The additional strategies will be documented foreach Ctient and kept in the Client's file(s). <br />(8) lf after twclve (12) months the Client remains unemployed, an additionalreview will be conducted. The provider will address steps oulined in theprevious six month progress report in the next six month progress report. TheClient may request to participate in Community lnclusion-activitres or theClient may choose to remain in an employment plogram. When requesting toparticipate in Community lnclusion services, tne Client shall communicate <br />directly with his or her DDA Case Manager. The DDA Case Manager isresponsible for authorizing community lnclusion services, <br />(9) For lndividual Employment where the service provider is also the Ctient,semployer long.term funding will remain available to the service provider /employer for six (6) months after the employee / DDA Client's date of hire.At the end of the six month period, if the DtiA Client continues to need <br />support on the job, another service provider who is not the employer ofrecord must provide the support unless the County issues prior written <br />approval for the service provider to continue to provide long-term supports ifneeded. <br />(10) For Group Supported Employment, Clients must have paid work or paid <br />training. The total number of direct service staff hours provided to the groupshould be equal to or greater than the group's collective amount of <br />individual support monthly base hours.lt tne direct service staff hours are <br />Professional Services Provisional Agreement <br />Page 23
