The Kittitas County Network will lead these efforts by utilizing a collective
impact modelthat includes shared measurement, continuous communication, and
backbone support for the collaboration.
Assessment
ln order to assess the clinical needs of inmates who are pre-screened by jail staff to be
candidates for treatment services, they will complete a full SUD Assessment with Merit
Resource Services. Each assessment is a verified tool based upon industry best
practices. These services will be available on all weekdays. Urine analyses and
consent to treatment documentation for SUD will be included in the assessment
process. The assessment services provided by Merit are in-kind and do not need to be
funded through this proposal.
Professional Services Agreement (rev. 0912412018)
Page 15of23
