EX lT *A" <br />SCOPE OF WORK <br />Kittitas County Health Network will provide Medication Assisted Treatment (MAT), <br />Substance Use Disorder (SUD) counseling and treatment services, and mental health <br />counseling io individuals both while they are in the Kittitas County Jail as well as upon <br />release. KCHN staff will lead this work through administration of subcontracts, tracking <br />and reporting of data related to performance and outcomes, convening the services <br />providers for continuous quality improvement efforts, and supporting post-release care <br />coordination efforts. MAT services for individuals in the jail and upon release will be <br />jointly provided by comprehensive Healthcare, community Heaith of central <br />Washington, and Kittitas Valley Healthcare. Merit Resource Services will provide SUD <br />assessments, counseling, peer support services for discharge planning and post- <br />release care coordination, and post-release treatment services. HopeSource will work <br />closely with the Hub and Spoke program staff, the peer support specialist, and KCHN <br />care coordinators to provide support in addressing social determinants of health <br />including but not limited to housing, transportation and work training during discharge <br />planning and post-release. <br />Kittit:ls ('oultr I lt'rlth Nctu or.!
