2.Treatment Information <br />3.Survey tool to assess the quality of treatment and self-reported changes in OUD <br />knowledge,confidence,and readiness for transition into the community. <br />4.Community transition and treatment retention rates. <br />5.Process notes such as annual summary reports,quarterly program monitoring meeting <br />notes,or contract deliverables to identify the successes and challenges of delivering the <br />program along with opportunities for improvement. <br />The Program Evaluation will communicate information regarding all aspects of the program <br />evaluation to staff to ensure clear expectations for the final program evaluation report.The <br />Program Evaluator will provide Kittitas County Sheriff's Office,the MAT partner,and BJA staff <br />with ample opportunity to provide insight and feedback to the report's methodology,format,and <br />focus. <br />Data Collection <br />The NCM and Peer Support Specialist will complete the intake assessment to establish <br />baseline data.Periodic benchmark data collection,via interviews or surveys,will be conducted if <br />all required data cannot be collected from partners.Table 2 provides a potential outline for <br />evaluation metrics and data collection. <br />Table 2.Potential Data Collection <br />Potential Program Metrics <br />•Percent of individuals screened for OUD <br />•Percent of Individuals diagnosed with OUD <br />•Percent of Individuals offered MAT <br />•Percent of Individuals who initiate MAT <br />•Average Length of Stay <br />•Percent of individuals who remain on MAT at the time of release to the community <br />•Percent of Individuals who have an appointment scheduled with a community MAT <br />provider when released <br />•Percent of Buprenorphine recipients given a bridging supply or prescription. <br />18 of 20
