EXHIBIT "A" <br />SCOPE OF WORK <br />The Contractor shall provide 100 hours of assistance to the department, to include <br />some of or all of the following areas: <br />• Public health emergency preparedness and response activities for COVID-19. <br />This may include surveillance, epidemiology, laboratory capacity, infection <br />control, mitigation, communications, community engagement, and/or other <br />preparedness and response activities for COVID-19. <br />• Collect, organize, analyze, enter, and report COVID-19 data, utilizing CREST, <br />WDRS, and internal tracking sheets. <br />• Work with partners to ensure testing is provided in a culturally and linguistically <br />responsive manner with an emphasis on making testing available to disproportionally <br />impacted communities and as part of the jurisdiction's contact tracing strategy. <br />• Maintain a current list of entities providing COVID 19 testing and at what volume. <br />• Work with partners to provide culturally and linguistically responsive community <br />outreach and education related to COVID-19. <br />• Regional active monitoring activities, including partnerships with WA DOH, other <br />Local Health Jurisdictions (LHJs) in the region to collaboratively support <br />epidemiologic and surge capacity needs, and ensure regular communication among <br />LHJs in the region. <br />Professional Services Agreement <br />Page 13 <br />
