Professional Services Agreement <br />Page 14 of 20 <br />EXHIBIT "A" <br />SCOPE OF WORK <br />The Contractor shall employ one or more Community Health Workers / Promotoras de <br />Salud, who will be a member of the Kittitas County Incident Management Team <br />(IMT) and report to the Operation Sections Chief, following the IMT structure. Job <br />description is as follows: <br />The COVID-19 Vaccine Outreach Community Health Worker (CHW) is a temporary <br />part-time position expected to last approximately three (3) months. The employee in this <br />position will specifically perform work with the Kittitas County Incident Management <br />Team (IMT) related to decreasing barriers to accessing COVID-19 vaccination for <br />Kittitas County’s Latinx and Spanish-speaking households. The position requires <br />courteous and cooperative interaction with all department and IMT staff, County, State, <br />and Federal agencies, and the general public, and will be expected to provide <br />assistance in other areas of Public Health as needed. All employees work under the <br />guidance of local/state/federal regulations. <br />IMPORTANT: This is position is expected to end July 26, 2021. Duties and hours may <br />be amended as determined by COVID-19 funding availability. <br />Essential Functions: <br />•Improve COVID-19 testing and vaccination access and uptake in local Latinx and <br />Spanish-speaking communities by: <br />•Identifying and implementing outreach strategies to reduce barriers to testing and <br />vaccination <br />•Increasing community COVID-19 knowledge <br />•Developing effective relationships with community members and community <br />partners <br />•Assisting with appointment scheduling <br />OTHER FUNCTIONS: <br />•Respond to Public Health related inquiries from the public as needed. <br />•Maintain confidentiality as described in agency, federal, and state required policies. <br />This job description is a summary of the primary functions and expectations for this <br />position. Additional duties may be assigned as required. <br />MINIMUM QUALIFICATIONS: <br />•Proficiency in verbal and written Spanish. <br />•Insight and knowledge of cultural norms in local Latinx communities.
