•Equipment Valued at $5000.00 Dollars (i.e.Engines,Electronics,etc.):Agencies are <br />required to update their inventory of durable items defined as having a cost or value of <br />$5000.00 dollars or more.Only items listed under "allowable expenses"defined in in the <br />document "Allowable Costs and Expenditures for State Vessel Registration Fees and <br />Federal Assistance Grants"(found on the MLE Forms Website at <br />http://mle.parks.wa.gov/)should be purchased with vessel registration fees. <br />o Equipment Type <br />o Manufacturer <br />o Model <br />o Year purchased <br />o Funds used for Purchase (local,state,federal) <br />Sale of Vessels Purchased with Vessel Registration Fees or STATE PARKS Provided <br />Federal Funding:AGENCY agrees to notify STATE PARKS 60 days in advance of the sale <br />date of any vessel purchased with federal funds provided by STATE PARKS.The sale of <br />vessels purchased with state dollars,when sold at the end of their useful life,must be consistent <br />with the AGENCY's policies and procedures.AGENCY agrees to remove the vessel from the <br />Patrol Vessel Inventory once it is sold. <br />Boating Accident Reports Required,Timeline to Submission:AGENCY agrees to submit <br />accident reports to STATE PARKS (in compliance with RCW 79A.60.200,RCW 79A.60.220 and <br />WAC 352-70)as follows: <br />.For any boating accident resulting in a death,or in an injury requiring hospitalization, <br />AGENCY agrees to: <br />o Notify STATE PARKS within 48 hours of becoming aware of the incident. <br />o Fill out and submit a complete Boating Accident Investigation Report (BAIR - <br />form number A-425)within ten days of the occurrence. <br />o Include the results of any other investigation conducted by the agency,including <br />but not limited to statements from witnesses or any party involved,photos,maps, <br />or additional information. <br />o Submit,within one week of completion,any coroner's reports concerning the <br />death of any person resulting from the boating accident. <br />•In addition to the requirements above,for any boating accident resulting in a death,or in <br />an injury requiring hospitalization,OR damage to any vessel or property of two thousand <br />dollars or more,OR a vessel is a complete loss,OR a person disappears from the <br />vessel under circumstances that indicate death,AGENCY agrees to submit a Boating <br />Accident Report (BAR -Form number A-440)within 10 days of the occurrence to <br />STATE PARKS. <br />Boater Assistance:AGENCY agrees to create and maintain the ability to respond,or <br />coordinate response to,boating emergencies that occur within AGENCY's jurisdiction and <br />document each occurrence and report it to STATE PARKS through the Summary of Activity <br />Report (SOAR)Web Form as prescribed in this agreement.AGENCY agrees to report each <br />incident utilizing the following definitions: <br />Updated 10/28/2020 Page 11 of 24
