The existing paint system has failed throughout much of the truss's
bottom chord, floorbeams, stringers and bearings (-20% of BMS paint element is in Condition State 3). There is
pack rust at multiple bottom chord panel points which would also be repaired during the paint project. The
asphalt plug joints at the ends of the truss and the poured rubber joints at the floorbeams have failed which are
contributing to the paint failure.
Anticipated repairs to the structure include: removal and replacement of paint system, repair and/or replacement
of deteriorated or damage steel truss members, replacement of expansion joints at the ends of the bridge,
replacement of sealant at existing concrete panel joints, and spall repairs at existing concrete piers.
Q Yes $86,434.18 ❑ No DBE Participation
Total Amount Authorized: $336,740.93
Q Yes $86,434.18 ❑ No MBE Participation
Management Reserve Fund: $25,000
❑ Yes © No WBE Participation
[I Yes No SBE Participation
0
Maximum Amount Payable: $361,740.93
Index of Exhibits
Exhibit A
Scope of Work
Exhibit B
DBE Participation
Exhibit C
Preparation and Delivery of Electronic Engineering and Other Data
Exhibit D
Prime Consultant Cost Computations
Exhibit E
Sub -consultant Cost Computations
Exhibit F
Title VI Assurances
Exhibit G
Certification Documents
Exhibit H
Liability Insurance Increase
Exhibit I
Alleged Consultant Design Error Procedures
Exhibit J
Consultant Claim Procedures
Agreement Number:
Local Agency A&E Professional Services Cost Plus Fixed Fee Consultant Agreement Page 1 of 14
Revised 02/01/2021
