The coordinated entry assessment includes a VI-SPDAT designed for <br />each demographic to be served to determine the level of vulnerability and to <br />develop a stability plan best suited to the needs of each client. <br /> <br />8. Administration <br />a. Grantees will submit the following monthly deliverables with completeness, <br />timeliness, accuracy and consistency: <br /> <br />i. Invoice and Voucher Detail Worksheet for reimbursement <br />(Guidelines: Fiscal Administration). <br /> <br />ii. Grantees commit to reporting complete, quality data that is timely, <br />truthful and accurate. (Guidelines: Requirements of all Lead Grantees <br />and Subgrantees Providing Direct Service and HMIS User <br />Agreement). <br /> <br />iii. Grantees shall comply with all of the requirements, policies and <br />procedures in the Shelter Program Grant Guidelines. <br /> <br />B. Performance <br /> <br />1. Requirements <br />a. Projects are not required to meet or make progress toward performance targets <br />as a condition of funding for the current contract period. Project performance <br />data will impact community and state level performance measures. <br /> <br />b. Grantees should aim to improve the housing outcomes of Shelter Program <br />participants.
