EXHIBI-tr"'"A" <br />SCOPE OF WORK <br />ln addition to providing all material and labor, the Contractor shall perform the following <br />services as detailed below upon Service Activation. Service Activation is defined as the <br />point in time in which the Contractor is notified by the County, in writing, that the <br />reservation of the block of rooms is needed to commence immediately, Service <br />Activation will be determined solely by County Health Officials relying on available data <br />and expertise of health officials demonstrating an imminent need for seryices, and once <br />services are activated, responsibilities of this contract will begin and will continue as <br />outlined below. Contractor shall have 24 hours' notice from time of Service Activation <br />notification for Contractor responsibilities to begin. <br />Contractor Responsibilities upon Service Activation <br />1. Contractor shall reserve exclusive use of 5 rooms of the Traveler's lnn at 1001 F <br />1st St, Cle Ef um, WA for use by guests referred by the County for a 30-day <br />period beginning at service activation. The rooms will consist of 4 single <br />occupancy rooms and 1 multiple occupancy room. The County shall retain the <br />right to extend its reservation of exclusive use of the 5 rooms for an additional 30 <br />days at a time. <br />2. Contractor shall provide one full time office employee on duty Monday through <br />Friday from 8:30am to 5:00pm, who will be responsible for the following: <br />a. stafl the office and respond to questions from guests, county, and <br />contracted service providers. <br />b. distribute and manage room keys to County staff or County designee <br />c. keep and manage daily registration list of guests <br />d. maintain list of unit occupancy <br />e. Contractor and Contractor employee's will not enter units while in use <br />except in the event of an emergency. <br />3. County staff or County Designee shall provide direct referrals to Contractor for <br />room occupancy. <br />4. Contractor will provide use of three sets of towels per unit per guest once per <br />week. <br />5. Contractor will provide use of two sets of bed linens per unit per bed once per <br />week. <br />6. Contractor will provide toilet paper, tissues, and soap for units as needed once <br />per week. <br />7. No food or beverage service will be provided. <br />8. Contractor shall allow service animals in designated units. <br />9. Contractor will allow County to provide onsite security services from the Kittitas <br />County Law Enforcement as deemed necessary by County. <br />Professional Services Agreement <br />Page 14 ot 20
