EXHIBIT "A" <br />SCOPE OF WORK <br />In addition to providing all material and labor, the Contractor shall perform the following: <br />® Implement the Community Prevention Wellness Initiative (CPWI) community- <br />based mentoring program. <br />o Produce 12 mentor/mentee matches by June 30, 2020. <br />o Work with the Cascade Prevention Coalition in developing and delivering <br />mentor services in the CPWI community. This includes recruitment of <br />mentors and mentees, mentor training, home visits, and coordination of <br />activities for matches. <br />Eligibility for mentees shall be determined based on application and <br />parent/guardian consent. <br />Eligibility for mentors is based on a seven fold background check <br />process, in home interview, and training. <br />o Regularly attend Cascade Prevention Coalition meetings. Collaborate <br />with coalition partners, including participation in community events as <br />appropriate. <br />o Participate in required DBHR training. <br />o Meet program delivery requirements using the Washington State Mentors <br />program model (as required by DBHR.) <br />o Complete reporting requirements in the Minerva system for each month by <br />the 15th day of the following month. <br />• Ensure accurate and unduplicated reporting <br />• Ensure proper training of staff and designated staff for back-up <br />Minerva data entry to meet report due dates. <br />• Ensure all required demographic information is provided for <br />individual participants, population reach, aggregate, environmental <br />and mentoring or 1 -to -1 services in Minerva. <br />o Media materials and publications developed shall be submitted to the <br />County for DBHR approval prior to publication (DBHR responds within 5 <br />working days). The Department of Social and Health Services (DSHS) <br />must be cited as the funding source in news releases, publication and <br />advertising messages created with or about DSHS funding. The funding <br />source shall be cited as Washington State Department of Social and <br />Health Services. The DSHS logo may also be used in place of the above <br />citation. Messaging specifically directed to youth does not require DSHS <br />logo or funding credit. The contractor does not need to receive prior <br />approval for newsletters and factsheets, news coverage resulting from <br />interviews with reporters including online news coverage, newspaper <br />editorials or letters to the editor, social media posts, statewide messages <br />Professional Services Agreement <br />Page 13 <br />
