(b) Ensures the individual receives services in the community to the same <br />degree of access as individuals not receiving Medicaid HCBS; <br />(c) The setting provides opportunities to seek employment and work in <br />competitive integrated settings; and <br />(d) The setting facilitates individual choice regarding services and supports, <br />and who provides them. <br />5. Quality Assurance & Evaluation. <br />a- County On-Site Evaluation and Review: The County shall evaluate and review <br />servides delivered to reasonably assure compliance and quality. The County <br />shall conduct at least one on-site visit to each sutlcuritractor durittg ilte <br />biennium. The County shall maintain written documentation of all evaluations, <br />recommendations and corrective action plans for each subcontractor, and <br />reviews of on-site visits. Copies of such documentation will be provided to the <br />DDA Office upon request. <br />6. Single State iledicaid Agency-DSHS. DSHS, as the single state Medicaici <br />Agency, has administrative authority for Title XIX coverage of services for people <br />with developmental disabilities per 42 CFR 431.10. Contractor only has <br />responsibility for services covered in this agreement. <br />7. DSHSTDRW Access Agreement: The DRW February 27 ,2OO1 Access Agreement <br />with the DDA is incorporated by reference. Contractor assures that it and its <br />subcontractors have reviewed the Access Agreement. The agreement @vers <br />DRWs access to individuals with developmental disabilities, clients, programs and <br />records, outreach activities, authority to investigate allegations of abuse and neglect, <br />other miscellaneous matters, and is binding for all providers of DDA contracted <br />services. <br />S. CMIS Data Systern. The Contractor shall use the CMIS data system for all billing <br />requests, service provider address and phone number maintenance, evaluation <br />dates and to provide employment outcome information. The Contractor shall: <br />a. Monthly provide all data described in the Billing lnstructions and in the <br />Employment Outcomes lnstructions incorporated by reference. <br />b. Assure integrity of data submitted to state. When data is submitted and <br />rejected due to errors or later an error is identified, Contractor will correct and <br />resubmit within thirty (30) days <br />Professional Services Agreement <br />Page 24
