<br />12/05/2017 MINUTES 3 <br /> <br /> <br /> <br />AGREEMENT COMMUNITY HEALTH OF CENTRAL WASHINGTON PUBLIC HEALTH <br /> <br />Approve a Professional Services Agreement between the Kittitas County <br />Public Health Department and Community Health of Central Washington, in <br />the amount of $13,258.00, to provide services for those eligible in the <br />Children with Special Health Care Needs program which is part of the <br />Maternal Child Health Block Grant, from October 1, 2017 through <br />September 30, 2018. <br /> <br />AGREEMENT WALLA WALLA COUNTY PUBLIC HEALTH <br /> <br />Approve an Agreement between Walla Walla County and the Kittitas County <br />Public Health Department, in the amount of $3,000.00 to participate in <br />the Greater Columbia Accountable Community of Health Youth Marijuana <br />Prevention and Education Program from July 1, 2017 through June 30, <br />2018. <br /> <br />RESOLUTION 2017-181 OFFICE CLOSURE PUBLIC HEALTH <br /> <br />Approve Resolution No. 2017-181, Authorizing the Closure of the Kittitas <br />County Public Health Department on December 15, 2017 for an annual <br />archiving day. <br /> <br />CONTRACT DEPARTMENT OF HEALTH PUBLIC HEALTH <br /> <br />Approve and authorize the Public Health Administrator to sign Amendment <br />Number 14 to Contract Number C17114 to the 2015-2017 Consolidated <br />Contract between the Department of Health and the Kittitas County Public <br />Health Department, in the amount of $11,049.00 for the FPHS Communicable <br />Disease & Support Capabilities and the Maternal & Child Health Block <br />Grant. <br /> <br />MEMORANDUM OF UNDERSTANDING FIRST UNITED METHODIST CHURCH PUBLIC HEALTH <br /> <br />Approve and authorize the Public Health Administrator to sign a <br />Memorandum of Understanding between the First United Methodist Church <br />and the Kittitas County Public Health Department to provide a space for <br />conducting harm reduction education, testing, counseling, and supply <br />distribution, beginning January 1, 2018 through December 31, 2022. <br /> <br />AGREEMENT WISE PUBLIC HEALTH <br /> <br />Approve a Professional Services Agreement between Kittitas County and <br />Wise, in an amount of $13,250.00 to provide individualized technical <br />assistance (ITA) services to qualified clients to address existing <br />barriers to employment from July 1, 2017 through August 31, 2017. <br />
