<br />08/02/2016 MINUTES 3 <br /> <br /> <br />AGREEMENT KITTITAS VALLEY OPPORTUNITIES PUBLIC HEALTH <br /> <br />Approve a Professional Services Agreement between Kittitas County and <br />Kittitas Valley Opportunities, dba Parent to Parent, in the amount of <br />$19,000.00, relating to the needs of children and adults diagnosed with <br />developmental disabilities from July 1, 2016 through June 30, 2017. <br /> <br />CONTRACT DEPARTMENT OF SOCIAL & HEALTH SERVICES PUBLIC HEALTH <br /> <br />Approve Amendment 2 to Contract Number 1536-44899 between the State of <br />Washington Department of Social and Health Services, Developmental <br />Disabilities Administration and Kittitas County, in the amount of <br />$738,941.00 from July 1, 2017 to June 30, 2017. <br /> <br />AGREEMENT BRIGHT BEGINNINGS PUBLIC HEALTH <br /> <br />Approve an Agreement between Kittitas County and Bright Beginnings of <br />Kittitas County, in the amount of $7,700.00 to provide information and <br />education to parents and the community in order to enhance early <br />learning and positive family growth from July 1, 2016 through June 30, <br />2017. <br /> <br />AGREEMENT CENTRAL WASHINGTON DISABILITY RESOURCES PUBLIC HEALTH <br /> <br />Approve a Professional Services Agreement between Kittitas County and <br />Central Washington Disability Resources, in the amount of $12,300.00 to <br />provide therapeutic information and resources that are not otherwise <br />available to children with disabilities in their families in Kittitas <br />County through the operation of a resource/lending center from July 1, <br />2016 through June 30, 2017. <br /> <br />AGREEMENT ELMVIEW PUBLIC HEALTH <br /> <br />Approve a Professional Services Agreement between Kittitas County and <br />Elmview to provide a coordinated and comprehensive local program of <br />services for persons with developmental disabilities, in an amount not <br />to exceed $595,732.00 from July 1, 2016 through June 30, 2017. <br /> <br />AGREEMENT ENTRUST COMMUNITY SERVICES PUBLIC HEALTH <br /> <br />Approve a Professional Services Agreement between Kittitas County and <br />Entrust Community Services to provide a coordinated and comprehensive <br />local program of services for persons with developmental disabilities, <br />in an amount $14,000.00 from July 1, 2016 through June 30, 2017. <br /> <br /> <br /> <br />
