Activity 630 — Dams (SDS): This credit is only applicable when the jurisdiction could <br />experience flooding from the failure of an upstream high -hazard -potential dam. <br />❑ For State Dam Safety Program (SDS) credit, provide a <br />description of the high -hazard -potential dam. <br />C Also provide a map showing the jurisdiction is within the <br />area of inundation of the high -hazard -potential dam. <br />Additional Notes: All letters, statement or written descriptions should be on the <br />jurisdictions letterhead and be signed by the appropriate staff or CRS Coordinator. <br />Maps produced for CRS Activities must include at least the following basic information: <br />A map title, the floodplain, street names, parcels and the map date.
