10/15/2019 MINUTES 4 <br /> <br /> <br />AGREEMENT PARENT TO PARENT PUBLIC HEALTH <br />Approve a Professional Services Agreement between Kittitas County and <br />Kittitas Valley Opportunities, dba Parent to Parent, in the amount of <br />$20,000.00 to provide emotional support, information and legislative <br />advocacy to families of children with developmental disabilities from <br />July 1, 2019 through June 30, 2020. <br /> AGREEMENT YAKIMA SPECIALTIES, INC. PUBLIC HEALTH <br />Approve a Professional Services Agreement between Kittitas County and <br />Yakima Specialties, Inc. to establish subcontracted employment and day <br />services for people with disabilities in Kittitas County from July 1, <br />2019 through June 30, 2020. <br /> AGREEMENT ENTRUST COMMUNITY SERVICES PUBLIC HEALTH <br />Approve a Professional Services Agreement between Kittitas County and <br />Entrust Community Services, to provide employment and day services for <br />people with disabilities in Kittitas County, at a rate of $75.00 per <br />service hour for Individual Employment; $53.00 per service hour for <br />Group Supported Employment and $35.00 per service hour for Community <br />Inclusion, from July 1, 2019 through June 30, 2020. <br /> AGREEMENT ELMVIEW PUBLIC HEALTH <br />Approve a Professional Services Agreement between Kittitas County and <br />Elmview, to provide employment and day services for people with <br />developmental disabilities in Kittitas County, at a rate of $75.00 per <br />service hour for Individual Employment; $53.00 per service hour for <br />Group Supported Employment and $35.00 per service hour for Community <br />Inclusion, from July 1, 2019 through June 30, 2020. <br /> AGREEMENT YAKIMA VALLEY MEMORIAL HOSPITAL/VIRGINIA MASON PUBLIC HEALTH <br />Approve a Professional Services Agreement between Kittitas County and <br />Yakima Valley Memorial Hospital Association dba Virginia Mason Memorial, <br />to provide needs of the general public, parents, and children or adults <br />diagnosed with developmental disabilities, in the amount of $10,000.00 <br />from July 1, 2019 through June 30, 2020. <br /> APPOINTMENT DAVID STURGELL COMMISSIONERS <br />Approve the appointment of David Sturgell to the Kittitas County <br />Homelessness and Affordable Housing Committee, as the Member-at-Large, <br />for a two year term effective immediately and expiring on December 31, <br />2021. <br />
