STATEMENT OF WORK AND BUDGET- SEE ATTACHED APPLICATION <br />REGARDING THE SENIOR SUPPORT AND ADVOCACY PROGRAM <br />REPORTING <br />For each Bi -Annual Report, the agency will use the standardized reporting form developed <br />by the Homeless and Affordable Housing Committee. <br />Quarterly Reports are due on or before the 15th day of each April, July, October and the <br />following January of each year of this Grant Agreement starting on April 15, 2020, with the <br />final report due on or before January 15, 2025. <br />Reports shall reflect the data compiled and provided in compliance with section 3.1. <br />Please send an electronic copy of each report to: <br />Kittitas County Homeless and Affordable Housing Committee <br />Taylor Crouch, Kittitas County Board of Commissioners office <br />Kittitas County Auditor's Office, Judy Pless <br />*Reporting requirements may be amended by the Committee so that appropriate data can be <br />gathered. <br />Homeless Housing Grant Agreement <br />Revised 12/2019 Page 10 of 10 <br />
