EXHIBIT "A" <br />SCOPE OF WORK <br />In addition to providing all material and labor, the Contractor shall perform the following: <br />The area to be repaired can be seen at the above address. <br />The cost proposal needs to address and include the following elements. <br />♦ Saw cut asphalt as needed. <br />♦ Remove and dispose of the existing asphalt mat, approx 6000 square feet. <br />Field verify, pavement has been marked. <br />♦ Excavate to a total depth of 15 inches, and dispose of the excavated material. <br />♦ Compact the exposed subgrade to the extent feasible. <br />♦ Place fabric across the exposed subgrade. (Mirafi 50OX or approved equal) <br />♦ Place 12 inches of Base Course (APWA Spec) <br />♦ Repave the area with 3" of Class "B" Mix in one lift (APWA Spec) <br />♦ Seal the paved area with emulsion at the rate of .1 gallon/SY <br />♦ Seal coat approx. 9000 square feet on existing asphalt at the rate of .1 <br />gallon/SY. <br />After completing the excavation, the county may determine to excavate deeper, and <br />place additional base course material. <br />The completed repair must match the existing condition closely to maintain drainage. <br />All work must be completed by October 31, 2019. <br />Professional Services Agreement (rev. 09/24/2018) <br />Page 13 of 17 <br />
