participate on work groups and task forces. They also advise the Board on trends and issues <br />impacting healthcare and implement Board directives on programs and services. <br />Work Groups & Task Forces are sub -groups reporting to the Leadership Council that plan and <br />implement strategies, engage community members, and track progress towards goals. Members <br />are often relevant subject matter or community experts. Work groups and task forces will be <br />created and dissolved by the Leadership Council and will report progress regularly to the Board <br />through the Executive Director and the Leadership Council representatives to the Board. Their <br />purpose is to plan and implement activities which carry out the mission of KCHN. <br />ROLES & RESPONSIBILITIES <br />All members agree to the following: <br />1. Fully participate and contribute positively and constructively to the mission, vision, and <br />goals of KCHN <br />2. Listen with an open mind and with the intent to understand <br />3. Share knowledge, opinions, perspectives, experiences, and expertise willingly <br />4. Offer potential solutions to problems and challenges <br />5. Respect and value the contributions of others <br />6. Engage in and support consensus based decision making <br />7. Share new funding opportunities and include in applications KCHN and/or KCHN members <br />when opportunities align with KCHN's mission, vision, goals, strategic plan, and/or <br />Community Health Improvement Plan <br />8. Commit to the values of KCHN (see Appendix B.) <br />9. Dedicate time for KCHN meetings <br />10. Treat shared information as confidential and agree not to disclose shared information to <br />unauthorized entities <br />11. Disclose any conflict of interest that may arise in the course of KCHN collaborative activities <br />12. Protect patients and comply with all Health Insurance Portability and Accountability Act and <br />federal requirements <br />BENEFITS OF COLLABORATION <br />Participation as a member of KCHN will enhance population health for our community and strengthen <br />health and social service systems. Additional member benefits will include the following: <br />1. Cross -sector collaboration through collaborative contracts and grants <br />2. Systems integration through collaborative community health assessment, community health <br />improvement plan, and care coordination <br />3. More equitable communities through collaborative care coordination <br />4. Healthier communities through collaborative information sharing <br />FUNDING <br />There is no funding provided under this MOU. <br />DURATION & TERMINATION <br />This MOU shall become effective upon signature by the duly authorized representatives of the Member <br />and KCHN and will remain in effect unless modified by mutual consent and executed in writing by the <br />authorized representatives of all parties.
