Enrollment Details <br />1. Enrolled Affiliate's Enterprise. <br />a. Identify which Agency Affiliates are included in the Enterprise. (Required) Enrolled Affiliate's <br />Enterprise must consist of entire offices, bureaus, agencies, departments or other entities of <br />Enrolled Affiliate, not partial offices, bureaus, agencies, or departments, or other partial <br />entities. Check only one box in this section. If no boxes are checked, Microsoft will deem the <br />Enterprise to include the Enrolled Affiliate only.
