"Covered entity" means a health plan, a health care clearinghouse or a health care provider <br />who transmits any health information in electronic form to carry out financial or administrative <br />activities related to health care, as defined in 45 CFR 160.103. <br />"CSAP" means SAMHSA's Center for Substance Abuse Prevention. CSAP works with <br />federal, state, public, and private organizations to develop a comprehensive prevention <br />system. <br />"Data" means information produced, furnished, acquired, or used by Contractor in meeting <br />requirements under this Agreement. <br />"DBHR" means the Division of Behavioral Health and Recovery or its successor. <br />"DEA" means United Stated Drug Enforcement Agency. <br />"Dedicated Marijuana Account" or "DMA" means revenue generated by the taxation of <br />retail marijuana as a result of the implementation of Initiative 502 (1-502) as authorized by <br />the Washington State Legislature in 2E2SHB 2136. <br />"DUNS" or "Data Universal Numbering System" means a unique identifier for <br />businesses. DUNS numbers are assigned and maintained by Dun and Bradstreet (D&B) <br />and are used for a variety of purposes, including applying for government contracting <br />opportunities. <br />"Effective Date" means the first date this Agreement is in full force and effect. It may be a <br />specific date agreed to by the parties; or, if not so specified, the date of the last signature of <br />a party to this Agreement. <br />"EPA" means the Environmental Protection Agency. <br />"Evidence -Based Program" or "EBP" means a program that has been tested in <br />heterogeneous or intended populations that can be implemented with a set of procedures <br />to all successful replication in Washington. An EBP has had multiple randomized and/or <br />statistically -controlled evaluations, or one large multiple -site randomized and/or <br />statistically -controlled evaluations, and the weight of the evidence from a systematic <br />review demonstrates sustained improvements in at least one of the desired outcomes. <br />"Fiscal/Program Requirements" means the Supplementary Instructions and Fiscal Policy <br />Standards for Reimbursable Costs otherwise known as the Billing Guide for Substance Use <br />Disorder Prevention and Mental Health Promotion and is located at: <br />htt s://www.hca.wa. ov/bjllers- roviders- artnersl rior-authorization-claims-and- <br />billin 1 rovider-billin - uides-and-fee-schedules#t. <br />"General Fund State" or "GFS" means the administrative allocation awarded <br />for Substance Abuse Block Grant. <br />Washington State Page 7 HCA Contract No K3924 <br />Health Care Authority <br />
