Attachment A - Statement of Work <br />1. Purpose <br />This agreement allows Kittitas County to collaborate with the Other Party to coordinate immunization events in one or more facilities <br />belonging to the Other Party. <br />2. Performance <br />Other Party will: <br />1. Provide an appropriate and adequate space free of charge for each immunization event. <br />2. Promote each immunization event. <br />3. Provide school staff support for each immunization event. <br />4. Coordinate with Kittitas County to provide volunteers to assist with each immunization event. <br />Kittitas County will: <br />1. Provide appropriate immunizations and supplies free of charge at each mobile immunization event. <br />2. Provide licensed nurses to screen and vaccinate students and other eligible individuals. <br />3. Provide other Kittitas County Public Health staff support for each immunization event. <br />4. Coordinate with the Other Party to provide volunteers to assist with each mobile immunization event. <br />
