<br />Professional Services Agreement (rev. 09/24/2018) <br />Page 13 of 17 <br />EXHIBIT "A" <br /> <br /> SCOPE OF WORK <br /> <br />In addition to providing all material and labor, the Contractor shall perform the following: <br /> <br />1) Remove and dispose of the existing asphalt/concrete. <br />2) Excavate to a total depth of 9 inches and dispose of the excavated material. <br />3) Compact the exposed subgrade to the extent feasible. <br />4) Place fabric across the exposed subgrade. (Mirafi 500X or approved equal) <br />5) Place 7” crushed rock Base Course and 2” crushed rock top course. <br />6) Repave the area with 3” of Class “B” Mix in one List. (APWA spec) <br />7) Seal the paved area with emulsion at the rate of .1 gallon/SY. <br />8) All work must be done between June 17, and June 27, 2019. <br /> <br />The completed repairs need to have slope away from buildings to existing drainage. <br />The areas colored in blue and ramp to gazebo (aqua blue) are included in repair. <br /> <br /> <br />
