PURPOSE <br />The purpose of this amendment is to provide additional funding of $1500.00 to expand and enhance <br />the original scope of work by purchasing signage to support smoke-free and vape-free policies in <br />Kittitas County. <br />PERIOD OF PERFORMANCE <br />The period of performance of this amendment shall commence March 1, 2019 and be completed by <br />June 30, 2019 unless terminated or suspended sooner as provided for herein. <br />STATEMENT OF WORK <br />Contractor agrees to continue to perform the activities listed in the original agreement and to purchase <br />signage to support smoke-free and vape-free policies in Kittitas County. <br />REIMBURSEMENT <br />Additional funds of $1500.00 will be added to the contract for a total of $5,560.00. The District will <br />reimburse consultant for actual expenses after the services have been performed, based on the <br />submittal of monthly-itemized invoices for good and services provided during the period of <br />performance. <br />All other terms and conditions of the original agreement remain in full force and effect. <br />BENTON-FRANKLIN HEAL TH DISTRICT <br />7102 W.
