Attachment A <br />ittitas County Health Network <br />Infrastructure <br />The purpose of the Kittitas County Health network (KCHN) is to improve population health through <br />cross -sector collaboration and systems integration. The role of each Network member is to adopt and <br />support the Network goals; and provide strategic guidance, vision, leadership, and oversight. The <br />Network will use "cascading levels of collaboration" to create a high degree of transparency among all <br />organizations and levels. Community members, agency partners, work groups, the backbone <br />organization, the Leadership Council, and the Board of Directors will engage in bidirectional public will, <br />execution, action planning, governance, vision, and strategy (i.e., information will flow both from the <br />top down and from the bottom up.) <br />Network members will participate at one of the following levels: <br />Board of Directors: comprised of cross -sector agency leadership (top level) representative of the <br />relevant ecosystem that provides high-level strategic direction for the Network and champions <br />its work. <br />* Leadership Council: comprised of cross -sector agency representatives and community members <br />who are highly engaged in leading community health improvement activities. <br />Work Croups & Task Forces: sub -groups reporting to the Leadership Council that plan and <br />implement strategies, engage community members, and track progress towards goals. Members <br />are often relevant subject matter or community experts. <br />Board of Directors Role: <br />Provide high-level strategic direction, financial oversight, and policy governance. Review and approve <br />organization's mission and purpose, policies, outcomes, programs, services, and financial controls and <br />health. Create a well-defined structure of effective Board committees with clear purposes and tasks. <br />Define and apply Articles of Incorporation and By -Laws. Select and evaluate the Executive Director. <br />Approval of all contracts and policies. Consistently seep sustainability and viability of the organization. <br />Support implementation of the Strategic Plan as outlined in the Community Health Improvement Plan. <br />Leadership Council Role; <br />Reports to the Board of Directors. Acts as a permanent advisory body through representation by three <br />designated and accepted non-voting members to the Board. Implement KCHN Strategy. Represent the <br />community in achieving the KCHN mission. Identify and recruit broad base of community members <br />including a significant number external to medical/behavioral/social service providers. Develop support <br />and buy in from diverse community sectors. Implement Community Engagement Plan. Implement <br />Communications Plan. Assess sustainability and viability. Lead vision and strategy development, <br />information sharing, community engagement and participation on work groups and task forces. Advise <br />Board on trends and issues impacting healthcare. Implement Board directives on programs and services. <br />Work Groups and Task Forces Role: <br />Work groups and task forces will be created and dissolved by the Leadership Council and will report <br />progress regularly to the Board through the Executive Director and the Leadership Council <br />representatives to the Board. Their purpose is to plan and implement activities which carry out the <br />mission of KCHN. <br />
