PURPOSE The purpose of this agreement is to support efforts to reduce initiation and use of tobacco and vapor products in the Greater Columbia Accountable Community of Health (GCACH) region through a comprehensive tobacco control program focusing on policy, systems and environmental changes. PERIOD OF PERFORMANCE The period of performance of this Agreement shall commence July 1, 2018 and be completed by June 30, 2019 unless terminated or suspended sooner as provided for herein. STATEMENT OF WORK Contractor agrees to perform the following activities throughout the duration of this agreement, • Coordinate and implement a policy ask related to SmokeNape Free Policy in your county in one or more of the following areas: ❑ Clean Air Fair ❑ Public Parks ❑ Public Housing []College Campus ❑ Individual policies for local business owners/food establishments • Continue to share updates regarding cessation resources with healthcare providers and community partners. • Incorporate TIPS Campaign into social medial postings and outreach events. • Coordinate with local youth and/or adults to conduct Standardized Tobacco Assessment for Retailer Settings (STARS) in local retail shops, or share data collected on previously conducted STARS Assessments with decision -makers and stakeholders (optional). + Submit monthly reports using the Monthly Reporting Template (Exhibit A) to the District's Program Contact (page 1) by the 10th of each month. Be subject to periodic audits by BFHD administrative staff. District agrees to provide the following support and activities throughout the duration of this agreement: V Provide all reporting templates and links a Provide technical assistance as needed • Provide STARS training materials if needed Provide complied STARS assessment data to Contractor and the GCACH region. o Provide voluntary policy materials for distribution to local businesses, food establishments and multi- unit housing property owners. REIMBURSEMENT The District will reimburse Contractor for allowable expenses based on monthly invoices submitted by Contractor to the District. Invoices must itemize specific hours and charges based on the budget previously submitted, not to exceed $4,060 for goods and services provided during the period of performance. Any charges based on an allocation method need to have an explanation for the basis of the cost approved by the District in advance. Submit invoices be the 20th of the following month by email, fax or mail to the District's contact listed on page 1 of this agreement.
