GOLDENDALE SCHOOL DISTRICT NO. 404 <br />APPLICATION FOR USE OF SCHOOL FACILITIES <br />INDEMNITY AGREEMENT <br />If application for use of school facilities is approved, applicant, by his/her signature, hereto agrees to <br />defend, indemnify and hold harmless the school district, its appointed and elected officials, and all <br />employees and agents from and against all claims, demands, loss or liability of any kind and <br />character, arising out of or in any way connected with the applicant's use of the school facilities <br />specified in the Application for Use of School Facilities and this agreement. <br />If requested, by the School District, the applicant shall provide school district with copies or <br />certificates of insurance including all appropriate and necessary endorsements prior to the time of <br />use by the applicant of the school facility for the purposes anticipated. <br />The School District may orally withdraw permission to use the school facilities in the event any <br />provision of this agreement and the attached Application for Use of School Facilities has not been <br />complied with or is violated, without liability of any kind to applicant. <br />In addition to the foregoing, applicant agrees to assume responsibility for and to reimburse the <br />school district for the costs of repair or replacement of any property damaged, destroyed or stolen <br />during the applicant's use of the facility hereinabove designated. <br />Applicant warrants that use of the school facilities will be strictly in accord with school policy and all <br />applicable provisions of state law. <br />Print Name of Applicant/Responsible Party <br />A <br />Signature: Applic nt/Responsible Party <br />L� <br />Date <br />
