DEFINITIONS FOR CHARS: <br />"Direct ldenttfler" means data or records that include; <br />• Patient first name; <br />• Patient middle name(s); <br />• Patient last name; <br />• Social Security number; <br />• Patient control number or medical record number; <br />• Patient zip code+ four digits; <br />• Dates that include day, month, and year; and <br />• Admission and discharge dates in combination. <br />"Potentially Identifiable Data File": means the data file that contains indirect patient <br />identifiers. Information from the file can likely be associated with the identity of the person or <br />persons to whom the data or records pertain when combined with other information. <br />"Indirect identifiers": means information that may identify a patient when combined with <br />other information. Identification of a specific patient is more likely when a file contains a group <br />of ten or fewer similar hospitalizations. Indirect patient identifiers include the following data <br />elements, in combination or individually, when they create a group of ten or fewer similar <br />hospitalizations in a file: <br />• Hospital or provider identifiers; <br />• Five digit zip code; <br />• County, state, and country of residence; <br />• Dates that include month and year; <br />• Admission and discharge hour; <br />• Secondary diagnosis, procedure, present on admission, external cause of injury, and <br />payer codes; <br />• Age in years; and <br />• Race and ethnicity. <br />GENERAL TERMS AND CONDITIONS <br />I. USE OF INFORMATION <br />The Information Recipient agrees to strictly limit use of information obtained or created <br />under this Agreement to the purposes stated in Exhibit I (and all other Exhibits <br />subsequently attached to this Agreement). For example, unless the Agreement specifies <br />to the contrary the Information Recipient agrees not to: <br />• Link information received under this Agreement with any other information. <br />• Use information received under this Agreement to identify or contact <br />ind ivid ua Is. <br />Page 6 of 26 <br />09/2017
