Request for Qualifications <br />Employment and Day Program Services <br /> <br /> <br />2018 Kittitas County RFQ 7 <br />• Within 60 days of the service authorization date, the Contractor shall have a current, fully signed, <br />County-approved Service Plan for a customer in accordance with DDA Criteria for Evaluation System. <br /> <br />• All services shall be provided and outcomes delivered according to the customer’s plan. Services <br />provided that are not identified in the plan shall not be compensated unless a written exception is <br />granted by the County. <br />• The Contractor shall provide progress reports every 6 months for all customers that describe <br />outcomes of plan activities to the customer, guardian, County, and DDA Case Manager. The reports <br />shall summarize the progress made towards the customer’s individualized goals, and will be due <br />within 30 days following the end of the six-month period. <br />• The Contractor shall: <br />o Review the customer’s employment goals, activities, and outcomes; <br />o Consult with the customer and/or the family/guardian; <br />o Develop additional strategies with the customer and/or family/guardian, County staff, <br />employment support staff and the DDA Case Manager to assist the customer in moving <br />towards employment; <br />o Provide a Progress Report to the customer/guardian, County, DDA Case Manager <br />outlining the results of the review, future changes in service delivery and strategies to <br />reach the employment goal; and <br />o Maintain all information in the customer’s file. <br /> For each six (6) month period that follows, the Contractor shall: <br />o Address steps outlined in the previous six (6) month Progress Report in the next six (6) <br />month report; <br />o Complete the review process and send a Progress Report as indicated above; and <br />o Maintain information in the customer’s file. <br /> Employment Services Requirements: Customers shall be supported to work towards a living wage. A <br />living wage is the amount needed to enable an individual to meet or exceed his or her living expenses. <br />Customers should average twenty (20) work hours per week or eighty-six (86) hours per month. <br /> Payment rates and terms are in compliance with DDA requirements and will be outlined in the <br />contract. Service Rates as of 1/1/2018: <br />o Individual Supported Employment: The hourly rate is $70.00. <br />o Group Supported Employment: The hourly rate is $50.00. <br /> <br />Community Access <br />Community Access Services (CA) are individualized services provided in typical integrated community settings <br />for individuals in retirement. Services will promote individualized skill development, independent living and <br />community integration for persons to learn how to actively and independently engage in their local <br />community. Activities will provide opportunities to develop relationships and to learn practice and apply skills <br />that result in greater independence and community inclusion. <br /> <br />CA Service Requirements: <br />• All services shall be delivered within Kittitas County. <br />• To ensure health and safety, positive image and relationships in the community, increase <br />competence and individualized skill building, and other expected benefits of Community Access,
